## Probable cause

AN INADVERTANT LANDING GEAR EXTENSION AND WHEELS DOWN WATER LANDING BY THE PILOT IN COMMAND. A FACTOR RELATING TO THE ACCIDENT WAS THE LACK OF CHECKLIST USE BY THE PILOT.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Nose over, landing - flare/touchdown
  - wheels down landing in water - inadvertent, pilot in command (cause)
  - checklist - not used, pilot in command (contributing factor)

## Analysis

PILOT OF AMPHIBIAN FLOAT DHC2 BEAVER DEPARTED AIRPORT ON WHEELS ON VFR FLIGHT TO VILLAGE SERVED BY SEADROME ONLY. PILOT COULD NOT RECALL RAISING WHEELS AFTER DEPARTURE. PILOT MADE NORMAL APPROACH TO WATER LANDING AT VILLAGE AND UPON TOUCHDOWN, AIRPLANE NOSED OVER INVERTED. PILOT RECALLED SEEING WHEELS EXTENDED FROM FLOATS AFTER SWIMMING FROM COCKPIT.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19921224051779A](https://openairdocs.org/incident/19921224051779A), Dec 24, 1992: Dhav DHC2*, Two Moon Bay, AK, Substantial damage. Nosed over landing amphibious craft in bay. Pilot beleives he forgot to retract gear for a water landing.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
