## Probable cause

THE FAILURE OF THE RIGHT BRAKE DUE TO IMPROPER INSTALLATION OF THE BRAKE LINE. FACTORS WERE THE PILOT'S IMPROPER IN FLIGHT PLANNING/DECISION BY NOT CHECKING THE BRAKES PRIOR TO LANDING, AND THE BERM.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Airframe/component/system failure/malfunction, landing - roll
  - landing gear,normal brake system - failure,partial (cause)
  - fluid,hydraulic - leak
  - in-flight planning/decision - improper, pilot in command (contributing factor)
  - maintenance,installation - improper, copilot/second pilot
- On ground/water encounter with terrain/water, landing - roll
  - terrain condition - berm (contributing factor)

## Analysis

AFTER TOUCHDOWN ON THE GRAVEL RUNWAY, THE PILOT APPLIED BRAKES AND FOUND THE RIGHT BRAKE INOPERATIVE. THE AIRPLANE VEERED OFF THE LEFT SIDE OF THE RUNWAY AND COLLIDED WITH A BERM. THE PILOT SAID HE DID NOT CHECK THE BRAKES PRIOR TO LANDING. POST ACCIDENT EXAMINATION FOUND THAT MOST OF THE HYDRAULIC BRAKE FLUID HAD DRAINED OUT OF THE RIGHT BRAKE SYSTEM FROM AN IMPROPERLY FLARED LINE AT THE RIGHT BRAKE ASSEMBLY.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19930502011569A](https://openairdocs.org/incident/19930502011569A), May 2, 1993: Piper PA18150, Birchwood, AK, Substantial damage. Right brake failed on rollout. Veered off runway. Hit berm. Brake line leaking at fitting. Lost all brake fluid.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
