## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of control - on ground/water, takeoff - roll/run
  - proper alignment - not maintained, pilot in command (cause)
- In flight collision with object, takeoff - initial climb
  - object - vehicle (contributing factor)
- Forced landing, landing - flare/touchdown

## Analysis

PILOT DID NOT MAINTAIN ALIGNMENT DURING TAKEOFF AND COLLIDED WITH A PILE DRIVER. THE AIRCRAFT WAS THEN LANDED GEAR UP INAN ADJACENT FIELD. THE WIND WAS CALM AND THERE WAS NO KNOWN REASON FOR FAILURE TO MAINTAIN RUNWAY ALIGNMENT.
