## Probable cause

INADEQUATE MAINTENANCE ADJUSTMENT OF THE LANDING GEAR SYSTEM WHICH RESULTED IN THE JAMMING OF THE LEFT GEAR DOOR.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Gear not extended, landing
  - landing gear,normal retraction/extension assembly - jammed (cause)
  - maintenance, adjustment - inadequate, company maintenance personnel (cause)

## Analysis

AFTER ARRIVING AT THE DESTINATION THE LANDING GEAR COULD NOT BE EXTENDED. THE AIRPLANE WAS FLOWN BACK TO ITS BASE WHERE A GEAR UP LANDING WAS PERFORMED. EXAMINATION OF THE AIRPLANE REVEALED THAT THE LEFT GEAR DOOR DID NOT UNLOCK. THE EMERGENCY EXTENSION SYSTEM FAILED TO OPERATE AS A RESULT OF EITHER THE FAILURE OF THE CLUTCH ACTUATOR TO RESET AFTER THE LAST RETRACTION OR SLIPPAGE OF THE CLUTCH. THE PILOT'S REPORT STATED THAT THE DIRECTOR OF MAINTENANCE INDICATED THAT THE ACCIDENT COULD HAVE BEEN PREVENTED BY MORE FREQUENT LUBRICATION OF THE CLUTCH.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19900123064649A](https://openairdocs.org/incident/19900123064649A), Jan 23, 1990: Mtsbsi MU2B20, Nashville, TN, Substantial damage. Unable to extend landing gear at destination. Returned. Landed gear up. Door actuator clutch slipped. Lacked lube.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
