## Probable cause

THE PILOT'S FAILURE TO MAINTAIN DIRECTIONAL CONTROL OF THE AIRPLANE DURING THE TAKE OFF ROLL.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of control - on ground/water, takeoff - roll/run
  - directional control - not maintained, pilot in command (cause)
  - weather condition - crosswind
  - airport facilities,runway/landing area condition - soft
- On ground/water encounter with terrain/water, takeoff - roll/run
  - terrain condition - ditch

## Analysis

THE PILOT REPORTED THAT DURING THE TAKE OFF ROLL THE AIRPLANE BEGAN VEERING TO THE LEFT. HE APPLIED RIGHT RUDDER, WHILE FULL RIGHT AILERON WAS APPLIED, TO CONTROL THE DRIFT. BECAUSE OF THE WIND GUST AND MUD ON THE SIDE OF THE RUNWAY THE DRIFT CONTINUED. THE LEFT WHEEL HIT A 'RUNWAY DRAG-SLED', SEPARATED FROM AIRPLANE AND THE AIRPLANE VEERED INTO A DITCH.
