## Probable cause

A non-mechanical loss of engine power during the takeoff roll due to fuel contamination. An additional cause was the pilot's inadequate preflight inspection. A factor in the accident was the soft ground.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of engine power(partial) - nonmechanical, takeoff - roll/run
  - fluid,fuel - contamination,water (cause)
  - preflight planning/preparation - inadequate, pilot in command (cause)
- Overrun, takeoff - aborted
  - terrain condition - soft
- Main gear collapsed, takeoff - aborted
  - terrain condition - soft (contributing factor)
  - landing gear,main gear - overload

## Analysis

The airplane was substantially damaged after it overran the end of the runway at a private strip and the left main gear collapsed following an aborted takeoff. The pilot stated that as he was halfway down the runway, he had a sudden loss of engine power. He decided to abort the takeoff, went to full reverse thrust, and applied the brakes. As he departed the end of the runway, the left main gear got caught in soft dirt and collapsed. Following the accident, the pilot reported that there was water found in the fuel.

Probable cause: The pilot’s failure to establish a proper airspeed and angle-of attack during takeoff in elevated density altitude and light tailwind conditions, which resulted in an aerodynamic stall.

Probable cause: The pilot's failure to properly set the engine power for takeoff, which resulted in the airplane subsequently settling into terrain.

Probable cause: The pilot's improper decision to takeoff with a tailwind which resulted in a collision with terrain immediately after liftoff. Factors contributing to the accident were the tailwind and the levee.
