## Probable cause

THE PILOT'S INADEQUATE PREFLIGHT INSPECTION, WHICH RESULTED IN THE GASCOLATOR DRAIN BEING LEFT OPEN.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of engine power(partial) - nonmechanical, takeoff - initial climb
  - fuel system,drain - open
  - aircraft preflight - inadequate, pilot in command (cause)
- Forced landing, descent - emergency
- On ground/water collision with object, landing - roll
  - object - other

## Analysis

THE PILOT STATED THAT THE AIRCRAFT ENGINE LOST POWER JUST AFTER LIFT-OFF, RESULTING IN A FORCED LANDING. THE AIRPLANE RAN OFF THE DEPARTURE END OF THE RUNWAY AND IMPACTED A LARGE ROUND BALE OF HAY. THE PILOT REPORTED THAT THE ENGINE WAS STILL RUNNING WHEN THE AIRPLANE CAME TO A STOP. THE ENGINE OPERATED NORMALLY DURING A POST-ACCIDENT ENGINE RUN. THE FAA INSPECTOR STATED HIS INVESTIGATION REVEALED THAT THE GASCOLATOR DRAIN VALVE WAS STUCK OPEN SLIGHTLY 'ALLOWING THE FUEL PUMP TO SUCK AIR ALONG WITH GAS.'

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19900712044489A](https://openairdocs.org/incident/19900712044489A), Jul 12, 1990: Piper PA28140, New Richmond, WI, Substantial damage. Engine lost partial power on takeoff. Struck large hay bale on forced landing.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
