## Probable cause

THE PILOT IN COMMAND'S FAILURE TO ATTAIN SUFFICIENT AIRSPEED TO ACHIEVE FLIGHT, AND HIS FAILURE TO ABORT THE TAKEOFF.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of control - on ground/water, takeoff - roll/run
  - terrain condition - dirt bank/rising embankment
  - airspeed - not attained, pilot in command (cause)
  - abort - not performed, pilot in command (cause)

## Analysis

THE AIRCRAFT WAS LOADED WITH A LIQUID PESTICIDE IN PREPARATION FOR AN AERIAL APPLICATION FLIGHT. ON TAKEOFF THE AIRCRAFT FAILED TO CLEAR A DIRT EMBANKMENT 200' BEYOND THE DEPARTURE END OF THE 2,090' RUNWAY. THE LOAD WAS LOST WHEN IT LEAKED OUT OF THE HOPPER AFTER IMPACT. EXACT LOADING WEIGHT AND BALANCE COULD NOT BE DETERMINED. NO MECHANICAL DEFECTS WERE FOUND DURING POST ACCIDENT EXAMINATION OF THE AIRCRAFT AND ENGINE.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19910527017509A](https://openairdocs.org/incident/19910527017509A), May 27, 1991: Piper PA25235, Neenah, WI, Substantial damage. Unable to lift on takeoff roll overran end of runway and struck embankment. Indicates over gross but load drained.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
