## Probable cause

THE PILOT'S FAILURE TO TAKE ADEQUATE REMEDIAL ACTION DURING THE ATTEMPTED CLIMB OUT. A RELATED FACTOR IS THE PREMATURE LIFT OFF.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of control - in flight, takeoff - initial climb
  - lift-off - premature, pilot in command (contributing factor)
  - airspeed(vs) - not maintained, pilot in command
  - remedial action - inadequate, pilot in command (cause)
- In flight collision with terrain/water, descent
  - terrain condition - open field
  - touchdown - abrupt, pilot in command

## Analysis

THE PILOT STATED HE WAS TAKING OFF WITH A LOAD OF WATER TO FLUSH OUT HIS AERIAL APPLICATION EQUIPMENT. THE AIRPLANE ROTATED, BUT THE PILOT REPORTED HE WAS UNABLE TO CLIMB OUT OF GROUND EFFECT. THE STALL WARNING HORN SOUNDED AND THE AIRPLANE SETTLED BACK TO THE GROUND, TOUCHING DOWN ABRUPTLY IN A NOSE HIGH ATTITUDE. THE PILOT STATED THERE WAS NO MECHANICAL MALFUNCTION OR LOSS OF POWER INVOLVED. HE STATED, 'I SHOULD HAVE KEPT THE AIRPLANE ON THE GROUND LONGER DURING THE TAKE OFF ROLL.'

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19910614022379A](https://openairdocs.org/incident/19910614022379A), Jun 14, 1991: Piper PA36375, Wahpeton, ND, Substantial damage. Rotated too soon on takeoff. Unable to climb, settled into adjacent field.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
