## Probable cause

AN IMPROPER MANEUVER AND FAILURE TO ATTAIN LEVEL OFF BY THE PILOT IN COMMAND WHILE PERFORMING AERIAL APPLICATION.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of control - in flight, maneuvering - aerial application
  - maneuver - improper, pilot in command (cause)

  - lack of total experience in type operation, pilot in command (contributing factor)
- In flight collision with terrain/water, descent - uncontrolled

## Analysis

WITNESSES NEAR THE SCENE OF THE ACCIDENT SAW THE AIRPLANE PERFORMING AERIAL APPLICATION OF AGRICULTURAL CHEMICALS TO A WHEAT FIELD. THEY SAW THE AIRPLANE COMPLETE A SWATH RUN AND THEN DESCEND INTO THE TERRAIN AS IT TURNED TO RETURN TO THE FIELD. A REVIEW OF THE PILOTS LOG BOOK REVEALED MINIMAL EXPERIENCE CONDUCTING AGRICULTURAL APPLICATION OPERATIONS.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19920517015259A](https://openairdocs.org/incident/19920517015259A), May 17, 1992: Piper PA25235, Iroquois, SD, Destroyed, 1 fatal. Wreckage indicates a stall during spray run turn around.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
