## Probable cause

THE PILOT-IN-COMMAND'S DECISION TO CONTINUE FLIGHT INTO KNOWN ADVERSE WEATHER. CONTRIBUTING TO THE ACCIDENT WAS THE ENCOUNTER WITH WEATHER CONDITIONS FAVORABLE FOR FORMATION OF AIRFRAME ICING.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- In flight encounter with weather, climb
  - weather condition - icing conditions (contributing factor)
  - in-flight planning/decision - poor, pilot in command
  - flight into known adverse weather - continued, pilot in command (cause)
- Loss of control - in flight, climb
  - wing - ice
  - ice/frost removal from aircraft - not possible, pilot in command
- In flight collision with terrain/water, descent - uncontrolled

## Analysis

THE PILOT FILED AN IFR FLIGHT PLAN TO CRUISE AT 10,000 FEET. WHEN HE RECEIVED PILOT REPORTS OF ICING BETWEEN 8,000 FEET AND 11,000 FEET HE REQUESTED AND WAS CLEARED TO CLIMB TO 12,000 FEET. DURING THE CLIMB HE REPORTED AN INABILITY TO HOLD ALTITUDE AND THE AIRCRAFT BEGAN A DESCENT. DURING THE DESCENT HE REPORTED LOSING CONTROL OF THE AIRCRAFT ON TWO OCCASIONS. THE AIRCRAFT WAS GIVEN ALTITUDES AND HEADINGS IN AN ATTEMPT TO GET THE AIRPLANE TO AN AIRPORT, HOWEVER THE AIRPLANE IMPACTED IN AN OPEN FIELD AND WAS DESTROYED BY IMPACT AND A PORT-IMPACT FIRE.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19911014045959A](https://openairdocs.org/incident/19911014045959A), Oct 14, 1991: Beech A36, Lakeville, MN, Destroyed, 2 fatal. Departed in known icing conditions aloft on IFR flight. Lost control after icing problem. Unable sustain flight.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
