## Probable cause

OPERATION OF AN AIRCRAFT WITH KNOWN DEFICIENCIES AND DETERIORATION/FAILURE OF THE MAGNETO(S). FACTORS RELATED TO THE ACCIDENT WERE: THE LACK OF AN ANNUAL INSPECTION AND NON-COMPLIANCE WITH AN AIRWORTHINESS DIRECTIVE (AD).

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of engine power(partial) - mech failure/malf, cruise
  - ignition system,magneto - deteriorated (cause)
  - maintenance, annual inspection - not performed, other person (contributing factor)
  - maintenance, compliance with ad - not performed, other person (contributing factor)
  - operation with known deficiencies in equipment - attempted, pilot in command (cause)
- Loss of engine power(total) - mech failure/malf, maneuvering - turn to landing area (emergency)
  - ignition system,magneto - failure,total (cause)
- Forced landing, descent - emergency
- Nose over, landing - roll
  - terrain condition - crop

## Analysis

PILOT EXPERIENCED A PARTIAL LOSS OF ENGINE POWER AND WAS ATTEMPTING A PRECAUTIONARY LANDING WHEN ENGINE QUIT TOTALLY. PILOT MADE FORCED LANDING IN CORN FIELD AND NOSED OVER DURING LANDING ROLL. INVESTIGATION REVEALED THAT PILOT WAS FERRYING AIRCRAFT FOR OWNER WHO HAD JUST PURCHASED IT. NO LOG BOOKS COULD BE FOUND. IT COULD NOT BE DETERMINED WHEN LAST ANNUAL INSPECTION HAD BEEN PERFORMED. ENGINE INVESTIGATION DETERMINED THAT THE RIGHT MAGNETO WAS INOPERATIVE, AND THAT THE LEFT MAGNETO WAS DETERIORATED, AND WOULD SHORT OUT AND FAIL WHEN IT WAS BROUGHT UP TO OPERATING TEMPERATURE. MAGNETO AD ISSUED IN 1973 HAD NOT BEEN COMPLIED WITH. THERE WAS NO RECORD OF ANY FERRY PERMIT REQUESTED.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19911007045199A](https://openairdocs.org/incident/19911007045199A), Oct 7, 1991: Piper PA12, Roscoe, IL, Substantial damage. Engine quit en route on unauthorized ferry flight. Nosed over in cornfield. Both mags bad.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
