## Probable cause

THE IN FLIGHT SEPARATION OF THE PROPELLER TIP. AN ASSOCIATED FACTOR IS THE ROUGH AND UNEVEN TERRAIN.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Airframe/component/system failure/malfunction, cruise - normal
  - propeller system/accessories,blade - separation (cause)
- Forced landing, descent - emergency
- On ground/water encounter with terrain/water, landing - flare/touchdown
  - terrain condition - rough/uneven (contributing factor)

## Analysis

THE PILOT REPORTED A SEVERE SHUDDERING SHORTLY AFTER DEPARTING THE TRAFFIC PATTERN. POST ACCIDENT INSPECTION REVEALED THAT 7 INCHES OF ONE PROPELLER TIP HAD SEPARATED IN FLIGHT, CAUSING SEVERE IMBALANCE AND VIBRATION. THE PROPELLER TIP SEPARATED AT A REPAIR SCARF JOINT GLUE LINE.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19920512014329A](https://openairdocs.org/incident/19920512014329A), May 12, 1992: Univar 1082, Olathe, KS, Substantial damage. Severe vibration on climbout. Shutdown engine. Struck a ditch on landing. Nosed over. Reworked prop tip broke off.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
