## Probable cause

THE INADEQUATE INSPECTION BY OTHER MAINTENANCE PERSONNEL, AND AN INADEQUATE PREFLIGHT INSPECTION BY THE PILOT IN COMMAND. A FACTOR CONTRIBUTING TO THE ACCIDENT WAS THE REVERSAL OF THE AILERON CABLES.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of control - on ground/water, takeoff - roll/run
  - flt control syst, aileron control cable/rod - incorrect (contributing factor)
  - maintenance, inspection - inadequate, other maintenance personnel (cause)
  - preflight planning/preparation - inadequate, pilot in command (cause)
  - directional control - not possible, pilot in command
- Nose over, takeoff - roll/run
  - ground loop/swerve - encountered, pilot in command

## Analysis

THE AIRPLANE WAS ON THE FIRST TAKEOFF AFTER AN ANNUAL INSPECTION AND REBUILD. THE AIRPLANE EXPERIENCED A LOSS OF DIRECTIONAL CONTROL, EXITED THE RUNWAY TO THE LEFT, AND NOSED OVER. AN EXAMINATION OF THE AIRPLANE AFTER THE ACCIDENT FOUND THE AILERON CABLES TO BE REVERSED.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19920523016059A](https://openairdocs.org/incident/19920523016059A), May 23, 1992: Aronca O58B, Oshkosh, WI, Substantial damage. Veered off runway and overturned on takeoff roll. Test flight after rebuild. Ailerons cables were reversed.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
