## Probable cause

FAILURE OF THE PILOT TO MAINTAIN DIRECTIONAL CONTROL DURING A TAKEOFF GROUND RUN.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of control - on ground/water, takeoff - roll/run
  - terrain condition - icy (contributing factor)
  - directional control - not maintained, pilot in command (cause)
- On ground/water collision with object, takeoff - roll/run
  - terrain condition - snowbank
- Nose over, takeoff - roll/run

## Analysis

THE ACCIDENT AIRPLANE COLLIDED WITH A SNOWBANK AND NOSED OVER WHILE ACCELERATING TO LIFTOFF FROM A TOUCH AND GO LANDING. THE STUDENT PILOT DESCRIBED HOW HE LOST DIRECTIONAL CONTROL WHEN THE AIRPLANE ENCOUNTERED A PATCH OF ICE ON THE RUNWAY.
