## Probable cause

THE PILOT'S FAILURE TO MAINTAIN CONTROL OF THE AIRPLANE.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of control - on ground/water, takeoff - roll/run
  - weather condition - crosswind (contributing factor)
  - terrain condition - soft (contributing factor)
  - aircraft control - not maintained, pilot in command (cause)
- Nose over, takeoff - roll/run

## Analysis

THE PILOT REPORTED HE WAS ATTEMPTING TO TAKEOFF FROM RUNWAY 36 WITH A WESTERLY CROSSWIND. SHORTLY AFTER LIFTOFF, THE AIRPLANE SETTLED BACK ON THE SOFT, WET, RUNWAY. THE PILOT WAS UNABLE TO MAINTAIN DIRECTIONAL CONTROL, THE AIRPLANE DRIFTED OFF THE EAST SIDE OF THE RUNWAY AND NOSED OVER. NO PREACCIDENT MECHANICAL ANOMALIES WITH THE AIRPLANE WERE CLAIMED OR DISCOVERED.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19930430015329A](https://openairdocs.org/incident/19930430015329A), Apr 30, 1993: Maule M5*, Green Bay, WI, Substantial damage. Attempted crosswind takeoff. Settled after liftoff on 50FT wet runway. Lost directional control. Exited, flipped.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
