## Factual narrative

On July 7, 2008, at 0357 local time, a Boeing 747-200FM, N714CK, operated by Kalitta Air as Centurion Airlines flight 164, was destroyed during a forced landing in Madrid, Cundinamarca, Colombia, shortly after taking off from El Dorado International Airport (SKBO), Bogota, Colombia. Of the eight on-duty and off-duty crew members onboard, five were seriously injured and three sustained minor injuries. On-ground injuries included two fatalities and one serious injury. Night visual meteorological conditions prevailed. Assisting were NTSB airworthiness and human performance specialists as well as technical advisors from the Federal Aviation Administration, Boeing Aircraft Company, Pratt & Whitney and the operator.
