## Probable cause

THE TOTAL FAILURE OF THE FLIGHT COMPARTMENT WINDSHIELD (COPILOT'S SIDE) FOR UNDETERMINED REASONS.

## Analysis

DURING CRUISE FLIGHT AT FLIGHT LEVEL 190, THE COPILOT'S WINDSHIELD FAILED CAUSING RAPID DECOMPRESSION. THE CAPTAIN DECLARED AN EMERGENCY AND LANDED THE AIRCRAFT WITHOUT INCIDENT. PORTIONS OF THE REMAINING WINDSHIELD WERE ANALYZED. THE WINDSHIELD WAS PROPERLY INSTALLED AND NO REASON FOR THE FAILURE COULD BE DETERMINED.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Incident 19910225017719I](https://openairdocs.org/incident/19910225017719I), Feb 25, 1991: Cessna 421C, Denver, CO, Minor damage. Co-pilots windshield failed at cruise altitude. Declared emergency and landed at Jeffco Airport.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
