## Probable cause

A PARTIAL LOSS OF ENGINE POWER ON INITIAL TAKEOFF CLIMB DUE TO A BENT AND LOOSE CARBURETOR HEAT CONTROL MECHANISM.

## Analysis

DURING INITIAL CLIMB, FOLLOWING TAKEOFF AFTER CONDUCTING SEVERAL TOUCH AND GO LANDINGS, THE ENGINE LOST PARTIAL POWER. FLIGHT COULD NOT BE MAINTAINED AND THE CERTIFIED FLIGHT INSTRUCTOR TOOK OVER CONTROL AND LANDED THE AIRPLANE IN A FIELD. DURING LANDING ROLL, THE AIRCRAFT ENTERED A FRESHLY PLOWED AREA WHICH WAS SOFT AND THE AIRPLANE NOSED OVER.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of engine power(total) - mech failure/malf, takeoff - initial climb
  - carburetor heat control,bellcrank - bent (cause)
  - carburetor heat control,linkage - loose (cause)
- Forced landing, descent - emergency
- Nose over, landing - roll
  - terrain condition - open field
  - terrain condition - soft (contributing factor)

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19911117052059A](https://openairdocs.org/incident/19911117052059A), Nov 17, 1991: Piper PA28161, Greeley, CO, Substantial damage. Engine lost power on climbout. Landed in soft field and nosed over. Carb heat cable kinked and loose. Heat half on.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
