## Probable cause

THE PILOT FAILED TO MAINTAIN CONTROL OF THE AIRCRAFT. A FACTOR WAS: IMPROPER SUPERVISION BY THE FLIGHT INSTRUCTOR.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of control - on ground/water, landing - flare/touchdown
  - aircraft control - not maintained, dual student (cause)
  - supervision - inadequate, pilot in command(cfi) (contributing factor)

## Analysis

WHILE THE CERTIFIED FLIGHT INSTRUCTOR WAS GIVING A COMMERCIAL PILOT A TAIL WHEEL CHECKOUT, THE AIRCRAFT OVERSHOT FINAL DURING A TOUCH-AND-GO LANDING. THE STUDENT WAS SLIPPING THE AIRCRAFT BACK TO LINE UP WHEN THE TAIL WHEEL CONTACTED THE GROUND. CONTROL WAS LOST AND THE AIRCRAFT FLIPPED OVER.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
