## Probable cause

THE STUDENT PILOT'S FAILURE TO MAINTAIN DIRECTIONAL CONTROL AND THE FLIGHT INSTRUCTOR'S INADEQUATE SUPERVISION.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of control - on ground/water, takeoff - roll/run
  - directional control - not maintained, dual student (cause)
  - supervision - inadequate, pilot in command(cfi) (cause)
- On ground/water encounter with terrain/water, takeoff - aborted
  - terrain condition - rough/uneven

## Analysis

DURING A TRAINING FLIGHT FOR THE SINGLE ENGINE RATED PILOT, THE CFI GAVE THE STUDENT A SIMULATED ENGINE FAILURE DURING THE TAKEOFF ROLL BY RETARDING POWER ON THE RIGHT ENGINE. FOLLOWING THE STUDENT'S CORRECT RESPONSE OF REDUCING POWER ON THE LEFT ENGINE, POWER WAS RESTORED TO BOTH ENGINES AND THE TAKEOFF ROLL WAS RE-INITIATED. DIRECTIONAL CONTROL WAS LOST AND THE AIRPLANE DEPARTED THE RIGHT SIDE OF THE RUNWAY AND COLLIDED WITH A PILE OF ROCKS. THE NOSE AND RIGHT MAIN LANDING GEAR WERE SHEARED.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19920302004819A](https://openairdocs.org/incident/19920302004819A), Mar 2, 1992: Beech 95B55, Longmont, CO, Substantial damage. Recovered from simulated engine out on takeoff roll. Added power to continue takeoff. Lost control ran off runway.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
