## Probable cause

The pilot’s failure to arrest the helicopter descent during the simulated engine out maneuver resulting in a hard landing.

## Findings

NTSB's coded findings; those marked are part of the probable cause.

## Analysis

The helicopter pilot stated that he was performing his first solo autorotation to a full landing. He began the maneuver at approximately 1000 feet, and near the ground he noticed that he had a higher than normal rate of descent. He flared late and the helicopter “settled fast”. As the helicopter touched down, the tail struck the ground resulting in substantial damage to the tail boom, tail rotor drive shaft, and tail rotor assembly. The pilot did not report any mechanical malfunctions or anomalies with the helicopter prior to the accident.

## Docket (1 files)

The files NTSB released with the investigation on December 11, 2009, on NTSB's docket system: https://data.ntsb.gov/Docket?ProjectID=75087. 413 KB in all.

- Pilot/Operator Aircraft Accident Report, NTSB Form 6120.1 (item 1, .PDF, 413 KB): https://data.ntsb.gov/Docket/Document/docBLOB?ID=40321984&FileExtension=.PDF&FileName=Pilot%2FOperator%20Aircraft%20Accident%20Report,%20NTSB%20Form%206120.1-Redacted.PDF

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
