## Probable cause

NON-INSTRUMENT RATED PILOT'S INTENTIONAL FLIGHT INTO AN AREA OF ADVERSE WEATHER ASSOCIATED WITH THUNDERSTORMS, HEAVY PRECIPITATION, AND TURBULENCE. THE PILOT EXPERIENCED VERTIGO AND LOST CONTROL OF THE AIRCRAFT. THE AIRCRAFT THEN SUSTAINED AN INFIGHT BREAKUP AT A HIGH ALTITUDE.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- In flight encounter with weather, cruise - normal
  - flight into known adverse weather - intentional, pilot in command (cause)
  - light condition - dark night (contributing factor)
  - weather condition - clouds (contributing factor)
  - weather condition - low ceiling (contributing factor)
  - weather condition - fog (contributing factor)
  - weather condition - turbulence (contributing factor)
  - vfr flight into imc - continued, pilot in command (cause)
- Loss of control - in flight, cruise
  - aircraft control - not maintained, pilot in command (cause)
  - spatial disorientation, pilot in command (cause)
  - lack of total instrument time, pilot in command (contributing factor)
- Airframe/component/system failure/malfunction, descent - uncontrolled
  - design stress limits of aircraft - exceeded, pilot in command (cause)
  - wing - overload
  - wing - separation
  - horizontal stabilizer attachment - overload
  - horizontal stabilizer attachment - separation
- In flight collision with terrain/water, descent - uncontrolled

## Analysis

THE NON-INSTRUMENT RATED PLT CONTD VFR FLT AT NIGHT INTO AN AREA THAT WX BRIEFERS & ATC CONTROLLERS HAD WARNED OF THUNDERSTORMS, HEAVY PRECIPATION & POSSIBLE TORNADO ACTIVITY. SUBSEQUENTLY, HE REQUESTED 'A LITTLE GUIDANCE' & SAID HE WAS HAVING TROUBLE 'HOLDING A POSITION' & WAS 'RUNNING ON A . . . MAGNETIC COMPASS.' SUBSEQUENTLY, THE ACFT ENTERED A STEEP DESCENT, THEN RADIO & RADAR CONTACT WERE LOST. THE ACFT BROKE UP IN FLT AT AN UNKNOWN ALT & WRECKAGE WAS SCATTERED OVER A 3 MI AREA. AN EXAM OF THE WRECKAGE REVEALED THE RGT WING HAD FAILED UNDER POSITIVE LOADS (UPWARD) &THE THE LEFT HORIZONTAL STABILIZER SPAR WAS BENT DOWNWARD. THE RGT HORIZONTAL STABILIZER SEPARATED & WAS NOT FOUND.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19880917045409A](https://openairdocs.org/incident/19880917045409A), Sep 17, 1988: Piper PA28R201T, Okmulgee, OK, Destroyed, 1 fatal. Warned not to proceed VFR flew into clouds and severe turbulence lost control and aircraft broke up in flight.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
