## Probable cause

THE INADVERTENT STALL OF THE AIRPLANE BY THE PILOT. FACTORS CONTRIBUTING TO THE ACCIDENT WERE THE PILOT'S DISREGARD OF THE WEATHER ADVISORY, THE UNFAVORABLE WINDSHIFT, AND THE THUNDERSTORM OUTFLOW.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- In flight encounter with weather, takeoff
  - weather condition - rain
  - touch-and-go - performed, pilot in command
  - weather condition - thunderstorm,outflow (contributing factor)
  - hazardous weather advisory - disregarded, pilot in command (contributing factor)
  - weather condition - unfavorable wind (contributing factor)
- Loss of control - in flight, takeoff - initial climb
  - airspeed(vs) - not maintained, pilot in command
  - stall - inadvertent, pilot in command (cause)
- In flight collision with terrain/water, descent - uncontrolled

## Analysis

THE AIRPLANE WAS CLEARED TO LAND ON RUNWAY 34R AND WAS ESTABLISHED ON FINAL, WHEN THE WINDS SHIFTED FROM 030 DEGREES AT 8 KNOTS TO 060 DEGREES AT 25 KNOTS. THE TOWER CONTROLLER HAD PREVIOUSLY ADVISED ALL TRAFFIC OF A LARGE THUNDER STORM ALONG THE EASTERN PERIMETER OF THE AIRPORT. THE CONTROLLER THEN ADVISED THE PILOT OF A WIND SHEAR ALERT. THE PILOT EXECUTED A TOUCH AND GO. THE WINDS SHIFTED AGAIN TO 090 DEGREES AT 20 KNOTS GUSTING TO 26 KNOTS. THE CONTROLLER ADVISED THE PILOT THAT HE COULD LAND ON RUNWAY 09. THE CONTROLLER THEN INSTRUCTED ALL AIRCRAFT TO GO AROUND AS THE WIND VELOCITY INCREASED TO 30 PLUS KNOTS. HEAVY RAIN SHOWERS WERE OCCURRING OVER THE AIRPORT. THE AIRPLANE WAS OBSERVED TO BE IN WHAT APPEARED TO BE SLOW FLIGHT. IT ROLLED TO THE LEFT AND STALLED, THEN IT IMPACTED THE GROUND AND BURNED.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19890625017319A](https://openairdocs.org/incident/19890625017319A), Jun 25, 1989: Cessna 152, Fort Worth, TX, Destroyed, 2 fatal. Departed with thunderstorms, windshear alerts. Lost control on a go around due to severe windshear. Vertical impact

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
