## Probable cause

PILOT'S FAILURE TO MAINTAIN DIRECTIONAL CONTROL. CONTRIBUTING TO THE ACCIDENT WAS THE PILOT'S DIVERTED ATTENTION.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of control - on ground/water, takeoff - roll/run
  - diverted attention, pilot in command (contributing factor)
  - directional control - not maintained, pilot in command (cause)
- On ground/water collision with object, takeoff - roll/run

## Analysis

THE STUDENT PILOT, ON A SUPERVISED SOLO FLIGHT, LOST CONTROL OF THE AIRPLANE, VEERED OFF THE RUNWAY, WENT OFF AN EMBANKMENT, AND IMPACTED THE TERRAIN, NOSE GEAR FIRST. THE STUDENT PILOT SAID HE FORGOT TO RAISE THE FLAPS DURING A TOUCH AND GO LANDING, AND HIS ATTENTION WAS DIVERTED WHEN HE LOOKED DOWN TO RAISE THEM.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19900719030719A](https://openairdocs.org/incident/19900719030719A), Jul 19, 1990: Cessna 150M, Fort Worth, TX, Substantial damage. Lost control raising flaps after power on for touch and go. Left runway, pulled mixture by mistake, went over dropoff.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
