## Probable cause

THE PILOT'S EXCEEDING THE AIRPLANE'S TAKEOFF PERFORMANCE CAPABILITY FOR THE CLIMATIC CONDITIONS.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Overrun, takeoff - roll/run
  - aircraft performance,takeoff capability - exceeded
  - planning/decision - poor, pilot in command (cause)
- On ground/water encounter with terrain/water, takeoff - roll/run

## Analysis

DURING TAKEOFF FROM AN UNIMPROVED AIRSTRIP THE AIRPLANE WOULD NOT LIFT OFF AND RAN OFF THE END OF THE RUNWAY. THE PILOT ATTEMPTED TO DUMP HIS LOAD OF CHEMICALS TO LIGHTEN THE AIRPLANE FOR THE TAKEOFF. THE AIR INTAKE FOR THE DUMP MECHANISM WAS FOULED BY GRASS AND BRUSH. THE PILOT STATED AIR TEMPERATURE WAS 'HOT' AND THE AIRPLANE FELT 'HEAVY.'

- [Accident 19910514016179A](https://openairdocs.org/incident/19910514016179A), May 14, 1991: Cessna A188B, Eudora, AR, Substantial damage. Lifted off few feet, settled back to runway and ran off end into mud and bushes. Flaps up and hot day were factors.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
