## Probable cause

A BLOCKED FUEL SCREEN WHICH RESULTED IN A FUEL STARVATION INDUCED LOSS OF POWER. A FACTOR WAS THE LACK OF SUITABLE TERRAIN FOR THE FORCED LANDING.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of engine power(total) - mech failure/malf, cruise
  - fuel system,screen - blocked(total) (cause)
  - fluid,fuel - starvation (cause)
- Forced landing, descent - emergency
- In flight collision with object, landing
  - terrain condition - none suitable (contributing factor)
  - object - tree(s)

## Analysis

WHILE RETURNING TO THE LOCAL AIRPORT TO RELOAD, THIS AERIAL APPLICATION AIRPLANE SUSTAINED A LOSS OF POWER. THE PILOT WAS UNABLE TO REACH A SUITABLE SITE AND COLLIDED WITH A TREE DURING LANDING. A BLACK POWDER-LIKE SUBSTANCE WAS FOUND BLOCKING THE FUEL SCREEN. NO SOURCE FOR THE CONTAMINATION WAS FOUND. FUEL SCREEN BLOCKAGE RESULTED IN THE FUEL STARVATION LOSS OF POWER.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19910719028949A](https://openairdocs.org/incident/19910719028949A), Jul 19, 1991: Scwzer G164B, Brinkley, AR, Substantial damage. Returning to base engine quit. Landed in trees. Black powdery substance blocked fuel screen. Origin unknown.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
