## Probable cause

THE PILOT'S FAILURE TO MAINTAIN DIRECTIONAL CONTROL OF THE AIRPLANE.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of control - on ground/water, landing - roll
  - rudder - inadvertent activation, pilot in command
- Loss of control - in flight, landing - aborted
  - directional control - not maintained, pilot in command (cause)
- In flight collision with terrain/water, descent - uncontrolled

## Analysis

DURING THE LANDING ROLL, THE PILOT'S FOOT SLIPPED OFF THE RIGHT PEDAL AND ONTO THE LEFT CO-PILOT'S PEDAL AND DIRECTIONAL CONTROL WAS LOST. THE PILOT APPLIED POWER AND THE AIRPLANE CLIMBED SLIGHTLY, ROLLED TO THE LEFT, AND DESCENDED UNCONTROLLED TO IMPACT THE TERRAIN NEXT TO THE RUNWAY. THE PILOT STATED NO MECHANICAL MALFUNCTION OCCURRED.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19910319006219A](https://openairdocs.org/incident/19910319006219A), Mar 19, 1991: Unipro MONOCOUPD145, Lafayette, LA, Substantial damage. Pilot reported his foot slipped off rudder on landing. Added power, lifted, rolled left and impacted nose first.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
