## Probable cause

THE INADVERTENT STALL BY THE PILOT. A FACTOR WAS THE IMPROPER DEPARTURE FLAP SETTING.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of control - in flight, takeoff - initial climb
  - flt control syst, wing flap control - improper (contributing factor)
  - stall - inadvertent, pilot in command (cause)
- In flight collision with terrain/water, descent - uncontrolled

## Analysis

THE PILOT WAS PERFORMING TOUCH AND GO MANEUVERS AT THE PRIVATE AIRSTRIP. THE AIRPLANE WAS OBSERVED TO LAND WITH FULL FLAPS AND THEN TAKE OFF. DURING THE INITIAL CLIMB, THE AIRPLANE'S WINGS APPEARED TO WOBBLE BEFORE THE NOSE DROPPED AND IT ROLLED TO THE LEFT IN A DESCENT. IT IMPACTED INTO A RESIDENCE'S YARD NEXT TO THE RUNWAY. THE PILOT/OWNER DIED AT THE HOSPITAL. THE EXAMINATION OF THE WRECKAGE REVEALED THE FLAPS WERE SET BETWEEN 15 AND 20 DEGREES AT IMPACT. NO MECHANICAL ANOMALIES WERE NOTED TO THE AIRPLANE OR ITS ENGINE. THE PILOT'S FAA MEDICAL HAD BEEN DENIED IN 1985.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19910911039319A](https://openairdocs.org/incident/19910911039319A), Sep 11, 1991: Cessna 150L, Collinsville, OK, Destroyed, 1 fatal. Stalled and crashed climbing out from a touch and go. Flaps were still down.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
