## Probable cause

THE LOSS OF ENGINE POWER DUE TO THE FRACTURE OF A FAN BLADE RESULTING FROM A MANUFACTURING DEFECT.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of engine power(partial) - mech failure/malf, takeoff - roll/run
  - 1 engine
  - abort - initiated, pilot in command
  - turbine assembly,turbine blade - fractured (cause)
  - material defect, manufacturer (cause)
- Miscellaneous/other, takeoff - aborted

## Analysis

THE TAKEOFF WAS ABORTED AS THE AIRPLANE WAS ACCELERATING THROUGH 90 KNOTS. EXAMINATION OF THE AIRPLANE REVEALED THAT THE NUMBER ONE ENGINE EXPERIENCED A FAILURE OF ONE FAN BLADE IN THE FAN DISK DUE TO A MANUFACTURING DEFECT.
