## Probable cause

THE PILOTS'DECISION TO FLY INTO MARGINAL VFR WEATHER. A FACTOR WAS THE GUY WIRE AND FOG.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- In flight collision with object, maneuvering - turn to reverse direction
  - object - guy wire (contributing factor)
  - flight into known adverse weather - performed, pilot in command (cause)
  - weather condition - fog (contributing factor)
- Loss of control - in flight, descent - uncontrolled
- In flight collision with terrain/water, descent - uncontrolled

## Analysis

A NON INSTRUMENT RATED PILOT DEPARTED FOR CROSS COUNTRY FLIGHT WITHOUT A PREFLIGHT WEATHER BRIEFING. SHORTLY AFTER DEPARTURE THE PILOT REPORTED THAT HE ENCOUNTERED A LOWERING CEILING AND INITIATED A 180 DEGREE RIGHT TURN. RESTRICTED VISIBILITY DUE TO FOG WAS REPORTED IN THE AREA. THE PILOT STATED THAT HE SAW THE GUY WIRE JUST PRIOR TO THE IMPACT. THE AIRPLANE DESCENDED UNCONTROLLED TO THE TERRAIN.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19920411012229A](https://openairdocs.org/incident/19920411012229A), Apr 11, 1992: Boeing A75N1, Lawton, OK, Substantial damage, 1 fatal. Departed into low ceiling and fog. Got worse, attempted to turn back and struck radio tower guy wire. Crashed.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
