## Probable cause

THE LOSS OF ENGINE POWER FOR UNDETERMINED REASONS. FACTORS WERE THE SMOKE IN THE COCKPIT AND LOSS OF OUTSIDE VISUAL REFERENCE.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of engine power, maneuvering - aerial application
  - reason for occurrence undetermined, pilot in command (cause)
- Forced landing, descent - emergency
- In flight collision with object, descent - emergency
  - fuselage,crew compartment - smoke (contributing factor)
  - clearance - not possible, pilot in command
  - visual lookout - not possible, pilot in command (contributing factor)

## Analysis

DURING SPRAYING OPERATIONS THE PILOT EXPERIENCED A POWER LOSS AND THE COCKPIT FILLED WITH SMOKE. AFTER THE LOSS OF OUTSIDE REFERENCES AND ENGINE POWER THE AIRPLANE DESCENDED UNCONTROLLABLY AND STRUCK A TELEPHONE POLE, FOLLOWED BY FURTHER DESCENT TO GROUND IMPACT. THE SMOKE SOURCE AND REASON FOR THE LOSS OF ENGINE POWER WAS UNDETERMINED.

## Factual narrative

On May 4, 1993, at 0900 central daylight time, a Rockwell S-2R, N424OX, was substantially damaged while maneuvering near Lookeba, Oklahoma. The commercial pilot, also the owner/operator, sustained minor injuries. Visual meteorological conditions prevailed.

During an interview with the pilot, conducted by a Federal Aviation Administration (FAA) inspector, he stated that he had completed a spray run and was maneuvering for the next pass when the cockpit filled with smoke and the engine lost power. He further reported that after he lost engine power his right wing struck a telephone pole and he descended uncontrollably to ground impact. The operator was unable to determine why the engine lost power. As a result of a bankruptcy proceeding the was removed from the warehouse and could not be located for additional inspection.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
