## Probable cause

CAPTAIN'S FAILURE TO FOLLOW FLIGHT MANUAL PROCEDURES IN THE OPERATION OF THE ANTI-ICE/DE-ICE SYSTEMS.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of engine power(total) - nonmechanical, landing - roll
  - flight manuals - not followed, pilot in command (cause)
  - anti-ice/deice system - improper use of, pilot in command (cause)
  - powerplant - ice (cause)

## Analysis

DURING THE DESCENT FOR LANDING THE AIRCRAFT ENTERED INSTRUMENT METEOROLOGICAL CONDITIONS AND ACCUMLUATED A SLIGHT COAT OF ICE. THE ENGINE ANTI-ICE/DE-ICE HEATERS WERE NOT ON THROUGHOUT THE DESCENT AND LANDING. ON TOUCHDOWN BOTH ENGINES FLAMED OUT AND THE AIRCRAFT WAS TOWED TO THE TERMINAL.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Incident 19880129071209I](https://openairdocs.org/incident/19880129071209I), Jan 29, 1988: Swrngn SA227AC, South Lake Taho, CA. Both engines flamed out on touchdown. Had entered icing conditions on approach without engine intake heat on.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
