## Probable cause

THE FIXED BASE OPERATOR MECHANIC USED IMPROPER BOLT/NUT DURING REPAIR OF THE ELEVATOR SYSTEM.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- In flight collision with object, cruise - normal
  - flt control syst, elevator control cable/rod - jammed (contributing factor)
  - maintenance, replacement - improper, company maintenance personnel (cause)
- Forced landing, descent - emergency
- On ground/water collision with object, landing - flare/touchdown
  - flare - not possible, pilot in command (contributing factor)

## Analysis

WHILE IN NORMAL CRUISE FLT, THE AIRPLANE'S ELEVATOR JAMMED. THE PILOT ELECTED TO LND AT THE NEAREST ARPT. THE PLT WAS UNABLE TO FLARE THE AIRPLANE AND IT SUSTAINED A HARD LANDING. THE INVESTIGATION DSCLSD THAT A BOLT IN THE ELEVATOR SYSTEM HAD BACKED OFF AND JAMMED THE ELEVATOR BELLCRANK. AN INCORRECT ELEVATOR RETAINING BOLT AND NUT WERE USED, WHICH DID NOT PROVIDE PROVISION FOR A COTTER PIN.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
