## Probable cause

THE TOTAL LOSS OF ENGINE POWER FOR UNDETERMINED REASONS, AND THE PILOT'S FAILURE TO MAINTAIN ADEQUATE ROTOR RPM DURING THE AUTOROTATION.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of engine power(total) - nonmechanical, cruise - normal
  - reason for occurrence undetermined (cause)
- Forced landing, descent - emergency
- In flight collision with terrain/water, descent - uncontrolled
  - autorotation - delayed, pilot in command (cause)
  - rotor rpm - not maintained, pilot in command (cause)
  - terrain condition - ground

## Analysis

ABOUT 4 MINUTES AFTER TAKING OFF FROM A HOSPITAL HELIPAD WHILE IN CRUISE FLIGHT AT ABOUT 150 TO 200 FEET AGL, THE HELICOPTER'S ENGINE LOSS POWER. THE HELICOPTER WAS OBSERVED MAKING AN IMMEDIATE LEFT TURN AND DESCENDING RAPIDLY. WITNESSES SAID THE HELICOPTER'S ROTOR BLADE WAS TURNING VERY SLOWLY. THE HELICOPTER COLLIDED WITH THE GROUND WITH VERY LOW ROTOR RPM FOLLOWING THE VERTICAL DESCENT. THE ENGINE AND ALL ACCESSORIES WERE EXAMINED IN DETAIL WITH NO MECHANICAL DEFICIENCIES NOTED. NO DEFICIENCIES WERE FOUND IN THE CONTROL SYSTEM, ROTOR SYSTEM OR CLUTCH FREE WHEELING UNIT.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19920508014059A](https://openairdocs.org/incident/19920508014059A), May 8, 1992: Robsin R22BETA, Anaheim, CA, Destroyed, 1 fatal. Lost power, autorotated to hard impact in gravel pit. No mechanical or pilot defect. Possible carburetor ice.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
