## Probable cause

THE PILOT'S INADEQUATE PREFLIGHT INSPECTION BY FAILING TO UNTIE THE FRONT SKID TIEDOWN ROPE.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Roll over, takeoff
  - landing gear, skid assembly - entangled (contributing factor)
  - aircraft preflight - inadequate, pilot in command (cause)

## Analysis

THE PILOT WAS BEGINNING A LOCAL FISH SPOTTING FLIGHT FROM A FISHING VESSEL. PRIOR TO DEPARTING HE FAILED TO UNTIE THE RIGHT FRONT SKID TIEDOWN ROPE DURING THE PREFLIGHT AND ATTEMPTED TO LIFTOFF WITH THE RIGHT FRONT SKID TIED DOWN. THE HELICOPTER ROLLED TO THE RIGHT AND THE MAIN ROTOR BLADES STRUCK THE FISHING VESSEL'S FLIGHT DECK.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19920805029139A](https://openairdocs.org/incident/19920805029139A), Aug 5, 1992: Hughes 269C, Pacific Ocean, Guam, Substantial damage. Failed to untie front skid. Rolled over on attempted liftoff. Main rotors struck fishing vessels deck.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
