## Probable cause

FAILURE OF THE INPUT DRIVE GEAR OF THE TAIL ROTOR GEAR BOX FOR UNDETERMINED REASONS BECAUSE THE DRIVE GEAR WAS NOT RECOVERED FROM THE OCEAN.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Airframe/component/system failure/malfunction, takeoff - initial climb
  - rotor drive system,tail rotor gearbox(90 deg) - undetermined
- Forced landing, descent - emergency
- In flight collision with terrain/water, descent - emergency
  - terrain condition - water

## Analysis

THE SIGHTSEEING HELICOPTERS HAD JUST TAKEN OFF AND WAS ABOUT 50 FT IN THE AIR WHEN TAIL ROTOR DRIVE AND CONTROL WAS LOST. THE PILOT EXECUTED A POWER ON FORCED LANDING TO THE OCEAN THE TAIL ROTOR GEAR BOX FAILED AT THE CASTING AROUND THE INPUT DRIVE GEAR AND THERE WERE SIGNS OF LOGJAMMING ON THE INTERIOR OF THE CASTING. THE DRIVE GEAR WAS NOT RECOVERED FROM THE OCEAN.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19900513018339A](https://openairdocs.org/incident/19900513018339A), May 13, 1990: Enstrm F28A, Marathon, FL, Substantial damage. Lost tail rotor control on climbout. Made power landing in ocean near shore. Tail rotor gear jammed.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
