## Probable cause

INSTRUCTOR PILOTS FAILURE TO ASSURE THAT THE FLAPS WERE IN THE PROPER POSITION FOR TAKEOFF RESULTING IN REDUCED CLIMB PERFORMANCE. THIS RESULTED IN THE FORCED LANDING, DURING WHICH, THE AIRPLANE COLLIDED WITH A POWER LINE, STALLED, THEN CRASHED TO THE GROUND.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Miscellaneous/other, takeoff - initial climb
  - raising of flaps - not performed, pilot in command(cfi) (cause)
  - proper climb rate - not possible, pilot in command
- In flight collision with object, descent - emergency
  - object - wire,static
- Loss of control - in flight, descent - uncontrolled
  - stall - inadvertent, pilot in command(cfi)
- In flight collision with terrain/water, descent - uncontrolled
  - terrain condition - ground
- Fire, other

## Analysis

THE INSTRUCTOR PLT STATED THAT AFTER PERFORMING 2 APCHS, EACH TO A FULL STOP LANDING, WHILE PREPARING TO TAKEOFF FOR A THIRD, HE RAISED THE FLAP SELECTOR HANDLE. AFTER DEPARTURE AT ABOUT 300 FT, HE REPORTED THAT THE ACFT WOULD NOT CLIMB. HE THEN SELECTED A FIELD FOR THE FORCED LANDING DURING WHICH, THE RT WING COLLIDED WITH A POWER LINE. THE ACFT STALLED & CRASHED IN THE FIELD. THERE WAS A MOMENTARY FIRE BUT IT EXTINGUISHED QUICKLY. HE FURTHER STATED THAT THE FLAPS WERE FOUND 20 DEGS DOWN BUT DOES NOT REMEMBER LOWERING THEM. THE ENG STARTED & OPERATED NORMALLY DURING THE POST-ACCIDENT ENG RUN.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19900630023109A](https://openairdocs.org/incident/19900630023109A), Jun 30, 1990: Cessna 150F, Pensacola, FL, Substantial damage. Encountered downdraft on departure, unable to recover. Struck wires, stalled and crashed. Had 20 degree flaps down.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
