## Probable cause

FAILURE OF THE PILOT TO AVOID A MUD HOLE ON THE RUNWAY DURING THE GROUND ROLL TO TAKEOFF.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- On ground/water encounter with terrain/water, takeoff - roll/run
  - distance - misjudged, pilot in command (cause)
  - terrain condition - soft
- Main gear collapsed, takeoff - roll/run

## Analysis

THE PLT STATED THAT DURING THE GROUND ROLL TO TAKEOFF, AFTER RAISING THE TAIL, THE RT SIDE OF THE ACFT BEGAN TO SINK. THE MAIN GEAR THEN BROKE OUT FROM UNDER THE ACFT CAUSING IT TO VEER OFF THE LT SIDE OF THE RWY. ACCORDING TO AN FAA INSPECTOR WHO EXAMINED THE AIRSTRIP, A MUD HOLE WAS LOCATED ON THE RWY AT THE LOCATION WHERE THE TAIL WHEEL WOULD BE LIFTED OFF. HE FURTHER STATED THAT THE PLT GOT TOO CLOSE TO THE SOFT SPOT & THE RT MAIN LANDING GEAR ATTACH BOLT BROKE AFTER THE GEAR TRAVELLED INTO THE MUD HOLE. METALLURGICAL EXAMINATION OF THE FAILED LANDING GEAR ATTACH BOLT REVEALED THAT IT FAILED DUE TO OVERLOAD.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19900921041739A](https://openairdocs.org/incident/19900921041739A), Sep 21, 1990: Cessna A188B, Vicksburg, MS, Substantial damage. Right main gear broke off on takeoff roll when it hit a mud hole in rough dirt Private strip.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
