## Probable cause

SEPARATION OF THE RIGHT MAIN LANDING GEAR DURING TAKE OFF DUE TO FAILURE OF THE BOLTS WHICH SECURE THE GEAR TO THE AIRFRAME. CONTRIBUTING TO THE ACCIDENT WAS THE FAILURE OF THE PILOT TO ABORT THE TAKEOFF.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Main gear collapsed, takeoff - roll/run
  - landing gear, main gear attachment - separation (cause)
- On ground/water encounter with terrain/water, takeoff - roll/run
  - terrain condition - ditch
  - aborted takeoff - not performed, pilot in command (contributing factor)

## Analysis

THE PLT REPORTED THAT DURING THE TAKEOFF THE RT WING DROPPED. HE CONTINUED THE TAKEOFF BUT THE ACFT TRAVELLED THROUGH TWO DITCHES BEFORE COMING TO REST. EXAM OF THE ACFT REVEALED THAT TWO OF THREE BOLTS WHICH SECURE THE RT MAIN LANDING GEAR TO THE AIRFRAME WERE NOT RECOVERED. THE BOLT THAT WAS RECOVERED WAS METALLURGICALLY EXAMINED WHICH REVEALED THAT IT FAILED DUE TO OVERLOAD. THERE WAS NO EVIDENCE OF OVER TORQUE.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19910914040109A](https://openairdocs.org/incident/19910914040109A), Sep 14, 1991: Cessna A188B, Holly Bluff, MS, Substantial damage. Right main landing gear separated on takeoff. Veered off strip. Hit 2 ditches. Attach bolts had failed from overload

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
