## Probable cause

THE FAILURE OF THE PILOT TO MAINTAIN ROTOR RPM AND THE FAILURE OF THE CFI TO MONITOR THE CONTROLS.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Airframe/component/system failure/malfunction, landing - flare/touchdown
  - misc rotorcraft,tail boom - failure,partial
  - collective - improper use of, pilot in command (cause)
  - monitoring - inattentive, pilot in command(cfi) (cause)

## Analysis

THE INSTRUCTOR PILOT SIMULATED AN ENGINE FAILURE AND THE PILOT EXECUTED A 180-DEGREE AUTOROTATION. DURING THE FLARE AND CUSHION THE PILOT ALLOWED THE ROTOR RPM TO DECAY AND THE INSTRUCTOR PILOT DID NOT MONITOR THE PILOT'S USE OF THE COLLECTIVE PITCH CONTROL. SHORTLY AFTER TOUCHDOWN A VIBRATION WAS FELT AND THE TAILBOOM FAILED.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19911006045089A](https://openairdocs.org/incident/19911006045089A), Oct 6, 1991: Bell 206L1, Hollywood, FL, Substantial damage. Practice engine out autorotation. On landing student failed to bottom collective causing tailboom to fall downward.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
