## Probable cause

FAILURE OF THE PILOT IN COMMAND TO CHECK THE FUEL TANK FOR CONTAMINANTS BEFORE DEPARTURE, WHICH RESULTED IN THE TOTAL LOSS OF ENGINE POWER DUE TO WATER CONTAMINATION.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of engine power(total) - nonmechanical, cruise - normal
  - fluid,fuel - contamination (cause)
  - aircraft preflight - improper, pilot in command (cause)
- Forced landing, descent - emergency
- In flight collision with object, descent - emergency
  - object - building(nonresidential)
- In flight collision with terrain/water, descent - uncontrolled

## Analysis

THE ACFT WAS REFUELED BEFORE DEPARTURE WITH THE ENG RUNNING BUT THE PLT DID NOT CHECK THE FUEL TANK FOR CONTAMINANTS. AFTER TAKEOFF DURING CRUISE FLT, THE ENG QUIT. THE PLT ATTEMPTED TO LAND ON A ROAD BUT THE ACFT COLLIDED WITH A BUILDING THEN THE GROUND. APRX 4 OUNCES OF FUEL WAS DRAINED FROM THE FUEL CONTROLLER OF WHICH 2 OUNCES WAS WATER.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19920307005539A](https://openairdocs.org/incident/19920307005539A), Mar 7, 1992: Ayres S2RT15RESTD, Hollindale, MS, Substantial damage. Engine quit. Collided with a building on forced landing. Found water in fuel from a field fuel truck.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
