## Probable cause

THE PILOT IN COMMANDS DECISION TO TAKEOFF FROM A ROAD WITH A LEFT QUARTERING TAILWIND WHICH RESULTED IN HIS LOOSING DIRECTIONAL CONTROL AND THE AIRCRAFT COLLIDING WITH A UTILITY POLE.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of control - on ground/water, takeoff - roll/run
  - weather condition - tailwind
  - weather condition - crosswind
  - wind information - disregarded, pilot in command (cause)
  - directional control - not maintained, pilot in command (cause)
- On ground/water collision with object, takeoff - roll/run
  - object - utility pole

## Analysis

THE PILOT STATED HE WAS TAKING OFF FROM A ROAD EN ROUTE TO PERFORM AERIAL APPLICATION. THE WIND WAS FROM THE LEFT REAR AT 10 TO 15 KNOTS AND DURING THE TAKEOFF HE LOST DIRECTIONAL CONTROL. THE AIRCRAFT VEERED TO THE LEFT AND THE LEFT WING HIT A UTILITY POLE.

## Factual narrative

On August 4, 1993, at 1430 central daylight time, an Ayres S2R-T15, N1950J, registered to Larry Pruden, collided with a utility pole during takeoff from a road while on a 14 CFR Part 137 aerial application flight. Visual meteorological conditions prevailed at the time and no flight plan was filed. The aircraft received substantial damage and the commercial rated pilot was not injured. The flight was originating at the time of the accident.

The pilot stated that he was departing from a road in the area where he was spraying. The winds were from the left rear at 10 to 15 knots, gusting. During the takeoff he lost directional control and the aircraft veered to the left were the left wing contacted a utility pole.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19930804027639A](https://openairdocs.org/incident/19930804027639A), Aug 4, 1993: Ayres S2RT15RESTD, Louise, MS, Substantial damage. Lost directional control on downwind takeoff from a road. Left wing contacted a utility pole.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
