## Probable cause

The pilots misjudgment of altitude during an attempted banner tow pick-up. This resulted in the banner becoming entangled around the main landing gear. The pilot made a go-around. On landing rollout the banner became entangled around a runway light, and the airplane nosed over inverted.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- In flight collision with object, maneuvering
  - object - other
  - altitude - misjudged, pilot in command (cause)
  - go-around - performed, pilot in command
- On ground/water collision with object, landing - roll
  - object - runway light
- Nose over, landing - roll

## Analysis

The pilot stated her first banner tow pick-up was unsuccessful, and received clearance from the tower for a second attempt. On the second attempt the banner became entangled around the left main landing gear. She made a go-around and landed on runway 25. The banner became entangled around a landing light, and the airplane nosed over.

## Factual narrative

On May 25, 1999, at about 1116 eastern daylight time, a Grumman G-164A, N711ZY, registered to Rosie O'Gradys Aviation Inc., operating as a 14 CFR Part 91 banner tow flight crashed on landing at Orlando Executive Airport, Orlando, Florida. Visual meteorological conditions prevailed and no flight plan was filed. The airplane sustained substantial damage. The airline transport pilot reported no injuries. The flight originated from the Orlando Executive Airport about 11 minutes before the accident.

The pilot stated she departed from runway 25 and remained in left closed traffic in preparation for a banner pick-up adjacent to runway 25. The first attempt was unsuccessful and she received clearance for a second attempt. During the second attempt the banner became entangled on the left main landing gear. She remained in left closed traffic and landed on runway 25. The banner drifted left, became lodged around a runway light, and the airplane nosed over inverted.
