## Probable cause

POWER PLANT FAILURE DUE TO A FRACTURE OF THE FIRST STAGE TURBINE WHEEL, RESULTING IN AN UNCONTROLLED AUTOROTATION.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of engine power(total) - mech failure/malf, approach
  - turbine assembly,turbine wheel - fractured (cause)
- Forced landing, landing - flare/touchdown
  - autorotation - uncontrolled, pilot in command (contributing factor)
- Hard landing, landing - flare/touchdown

## Analysis

THE PILOT WAS ON AN APPROACH TO A FIELD SITE WHEN HE HEARD A LOUD 'BANG' FOLLOWED BY AN ENGINE OUT LIGHT AND AUDIO WARNING. THE PILOT EXECUTED A FORCED LANDING, WITH LOSS OF TAIL ROTOR CONTROL, WHICH RESULTED IN A HARD LANDING. TEARDOWN AND EXAMINATION OF THE ENGINE REVEALED THAT THE FIRST STAGE TURBINE WHEEL FRACTURED DUE TO THERMAL FATIGUE CRACKING.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19890804037079A](https://openairdocs.org/incident/19890804037079A), Aug 4, 1989: Bell 206B, Sebeka, MN, Substantial damage. Engine failed at low speed and altitude on landing approach. Crashed. Turbine failure.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
