## Probable cause

FAILURE OF THE ENGINE FUEL PUMP.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of engine power, takeoff - roll/run
  - fuel system,pump - failure,total (cause)
- On ground/water encounter with terrain/water, takeoff - aborted
  - terrain condition - ditch (contributing factor)

## Analysis

THE PLT RPRTD THAT DRG THE TAKEOFF ROLL FOR AN AERIAL APPLICATION OPN, THERE WAS A LOSS OF ENG POWER. BEFORE STOPPING, THE ACFT COLLIDED WITH A DITCH, TEARING OFF THE TAIL & SUSTAINING ADDITIONAL SUBSTANTIAL DAMAGE. A POST-ACDNT INVESTIGATION REVEALED THE ENG FUEL PUMP HAD FAILED.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19900403007469A](https://openairdocs.org/incident/19900403007469A), Apr 3, 1990: Ayres S2RR3SRESTD, Des Arc, AR, Substantial damage. Engine lost power on liftoff. Landed. Ran off end dirt runway into a ditch. Fuel pump had failed.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
