## Probable cause

FUEL EXHAUSTION INDUCED POWER LOSS CAUSED BY THE PILOT'S FAILURE TO REFUEL THE AIRPLANE.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of engine power(total) - nonmechanical, maneuvering - aerial application
  - fluid,fuel - exhaustion (cause)
  - refueling - not performed, pilot in command (cause)
- Forced landing, descent - emergency
- In flight collision with terrain/water, landing
  - terrain condition - none suitable (contributing factor)

## Analysis

THE COMMERCIAL PLT SAID THAT WHILE CONDUCTING AN AERIAL APPLICATION OPERATION, A LOSS OF FUEL PRESSURE OCCURRED, FOLLOWED BY A LOSS OF ENGINE POWER. AN EMERGENCY LNDG WAS MADE IN A FARM FIELD, WHICH RESULTED IN SUBSTANTIAL AIRCRAFT DAMAGE. POST-ACCIDENT INVESTIGATION REVEALED THE FUEL SYSTEM WAS EMPTY OF USABLE FUEL.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19900616023579A](https://openairdocs.org/incident/19900616023579A), Jun 16, 1990: Wthrly 201B, Stuttgart, AR, Substantial damage. Lost power on spray run after loss of fuel pressure. Landed farm field. Fuel system empty of usable fuel.

## Other investigations on N1260W (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB FTW91DPG10](https://openairdocs.org/ntsb/FTW91DPG10), May 23, 1991: Weatherly 201B (N1260W), Mc Crory, AR, substantial damage. Probable cause: LOSS OF THE OUTBOARD PORTION OF THE PROPELLER BLADE. A FACTOR WAS THE LACK OF SUITABLE TERRAIN FOR THE FORCED LANDING.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
