## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- In flight collision with terrain/water, takeoff - initial climb
  - touch-and-go - initiated, dual student
  - raising of flaps - not performed, dual student (contributing factor)
  - lift-off - initiated, dual student
  - gear retraction - premature, dual student (contributing factor)
  - proper climb rate - not attained, dual student (cause)
  - supervision - inadequate, pilot in command(cfi) (cause)
  - propeller system/accessories,blade - overload
  - propeller system/accessories - vibration
  - aborted takeoff - performed, pilot in command(cfi)

## Analysis

DURING THE TAKEOFF PORTION OF A TOUCH-&-GO, THE STUDENT PLT FORGOT TO RAISE THE FLAPS. THE INSTRUCTOR (CFI) YELLED AT THE STUDENT '...THE FLAPS', & AT THE SAME TIME, REACHED FOR THE FLAPS WITH HIS RIGHT HAND UNDER THE CONTROL WHEEL. SUBSEQUENTLY, THE LEFT PROP HIT THE SURFACE AT ABOUT THE SAME TIME A CLIMB WAS ATTEPMTED AT THE BLUE LINE SPEED. ALSO AT ABOUT THAT SAME TIME, THE CFI NOTED AN IN-TRANSIT GEAR LIGHT. THE CFI NOTED A SEVERE VIBRATION & ABORTED THE TAKEOFF. THE ACFT'S PROPS WERE DAMAGED & ITS UNDERSIDE WAS SCRAPED.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19840317005789A](https://openairdocs.org/incident/19840317005789A), Mar 17, 1984: Beech 76, West Mifflin, PA, Substantial damage. Pilots lost control of aircraft during touch and go when student forgot the flaps and instructor tried to correct.

## Other investigations on N6697L (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR12LA128](https://openairdocs.org/ntsb/WPR12LA128), Mar 11, 2012: Beech 76 (N6697L), Rio Linda, CA, substantial damage. Probable cause: A loss of engine power due to carburetor icing, and the pilot's failure to use carburetor heat during descent while operating in an area conducive to carburetor icing.
