## Probable cause

THE PILOT'S DELAY IN REMEDIAL ACTION BY DELAYING THE PULL-UP FROM A DESCENDING MANEUVER.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- In flight collision with terrain/water, maneuvering - turn to reverse direction
  - remedial action - delayed, pilot in command (cause)
  - pull-up - delayed, pilot in command (cause)

## Analysis

THE HELICOPTER ENTERED A SHALLOW CLIMB & STEEP TURN AT LOW ALTITUDE. ALTHOUGH THE PILOT INITIATED A RECOVERY, THE HELICOPTER STRUCK THE GROUND BEFORE HE RECOVERED. AN INVESTIGATION REVEALED NO PRE-EXISTING FAILURE OR MALFUNCTION WITH THE ENGINE OR AIRFRAME. A PERFORMANCE STUDY SHOWED THE MANEUVER HAD A MAXIMUM BANK OF 98 DEGREES & A MAXIMUM ALTITUDE OF 75 FEET AGL. RECOVERY WAS NOT INITIATED UNTIL .5 SECONDS AFTER THE APEX OF THE MANEUVER.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19890920035629A](https://openairdocs.org/incident/19890920035629A), Sep 20, 1989: Bolkms BO105C, Tuckerton, NJ, Destroyed, 1 fatal. Acrobatic practice. Failed to timely recover from negative g climb and turn apex. Had begun recovery at impact.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
