## Probable cause

THE NON-INSTRUMENT RATED PILOT INITIATING VFR FLIGHT INTO IMC ON A DARK NIGHT WHICH RESULTED IN SPATIAL DISORIENTATION AND UNCONTROLLED FLIGHT INTO THE GROUND. CONTRIBUTING FACTOR(S) WAS: THE ADVERSE WEATHER EXISTING AT THE TIME OF THE ACCIDENT AND THE PILOT'S SELF-INDUCED PRESSURE TO GET TO WORK.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- In flight encounter with weather, maneuvering
  - weather condition - low ceiling (contributing factor)
  - weather condition - rain (contributing factor)
  - weather condition - drizzle/mist (contributing factor)
  - weather condition - fog (contributing factor)
  - preflight planning/preparation - inadequate, pilot in command (contributing factor)
  - vfr flight into imc - initiated, pilot in command (cause)
  - self-induced pressure, pilot in command (contributing factor)
- Loss of control - in flight, maneuvering
  - light condition - dark night (contributing factor)
  - aircraft control - uncontrolled, pilot in command (cause)
  - spatial disorientation, pilot in command (cause)
- In flight collision with terrain/water, descent - uncontrolled

## Analysis

A FRIEND OF THE PILOT REPORTED THAT IT WAS RAINING AND FOGGY AS SHE DROVE HIM TO THE DEPARTURE AIRPORT. SHE ALSO SAID THAT WORK WAS VERY IMPORTANT TO THE PILOT, AND THAT HE FELT THAT HE HAD TO GET TO WORK. WITNESSES REPORTED THAT AT THE TIME OF THE ACCIDENT IN THE IMMEDIATE AREA, THERE WAS WIND, RAIN OF VARYING INTENSITY, LOW CLOUDS AND REDUCED VISIBILITYAVERAGING BETWEEN 1/4 AND 1 MILE.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19891128047429A](https://openairdocs.org/incident/19891128047429A), Nov 28, 1989: Piper PA32260, Coventry, CT, Destroyed, 1 fatal. Rain and fog. Felt need to go due to job. Rain fog near zero in crash area. Spatial spiral dive.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
